Toronto Fertility fertility blood work
 


Prior to your appointment, we will need certain medical information from you and your partner. We will also require certain medical information from your referring physician. In the event you are coming to TCART without a referral, we will provide you with laboratory requisitions outlining the tests and the results that we require before your first appointment.

The following medical information is required prior to your first appointment at TCART:

  • Letter from your referring physician (if you are referred)

  • Any pertinent operative reports

  • Results of tubal dye injection test (Hysterosalingogram, or HSG)

  • Required Blood Work:

    Female* - Hepatitis B&C, Hep B SAg, Day 3 FSH, Prolactin, Cytomegalovirus, HTLV I, HTLV II, HIV-I and HIV-II, TSH, Blood Film Exam, Rubella titre, Prenatal: ABO, Rhd, Antibody Screen, VDRL
    *Female blood work should be drawn on the third day of your menstrual bleed.
    Male - Hepatitis B&C, Hep B SAg, Cytomegalovirus, HTLV I, HTLV II, HIV-I and HIV-II, Prenatal: ABO, Rhd, VDRL
    For all patients using donor sperm a urethral swab is required for Chlamydia for the male partner and an endocervical swab for Chlamydia on the female partner prior to treatment at the clinic. Please have this test done at your family physician and the results sent to TCART.
